PEMF vs Red Light Therapy: How They Differ and What the Evidence Shows
PEMF vs red light therapy compares two different kinds of energy: PEMF uses electromagnet coils to create changing magnetic fields, while red light therapy shines red and near-infrared light onto the skin. Both have placebo-controlled reviews for arthritis pain, and PEMF also has an established medical use for fractures that fail to heal. Below: how each works, the knee evidence side by side, bones, surgery, using both, session lengths, safety and where our panel and mask fit.
Key takeaways
- A Cochrane review of 9 trials with 636 adults found PEMF gave 15.1 points more pain relief than placebo on a 100-point scale in osteoarthritis. Cochrane, 2013
- A 2019 review of 22 placebo-controlled trials with 1,063 people found knee pain 14.2 mm lower with laser light than with placebo on a 100 mm scale, and 18.7 mm lower at recommended doses. Stausholm, 2019
- In the one trial that compared PEMF with laser light in knee arthritis, pain and physical function improved in both groups after six sessions over three weeks. Elboim-Gabyzon, 2023
- PEMF is approved as a medical treatment for specific conditions, including fractures that do not heal with standard treatment. NCCIH, 2023
- The pulse setting on a red light panel switches the light on and off, and it does not create a magnetic field. Pulsed vs continuous light
PEMF vs red light therapy: how do they differ?
They use different energy: PEMF sends pulses of electromagnetic waves through the body, while red light therapy uses light that cells in and just under the skin absorb. Different energy means different devices and separate evidence, so a result for one does not carry over to the other.
Cleveland Clinic describes PEMF as a complementary therapy that uses "low-level pulses of electromagnetic waves". Cleveland Clinic, 2026 It usually comes as a mat you lie on or a smaller pad for one area.
Red light therapy, also called photobiomodulation, is thought to act on the mitochondria in cells. Proposed effects include more collagen, more fibroblasts, better blood flow and less inflammation. Cleveland Clinic, 2021 Clinic cold lasers and home LED devices belong to the same family; see cold laser therapy vs red light therapy.
| PEMF | Red light therapy | |
|---|---|---|
| Energy | Changing magnetic fields from electromagnet coils | Visible red and invisible near-infrared light |
| Typical device | A mat you lie on, or a pad for one area | LED panel, LED or laser face mask, clinic laser |
| How it reaches tissue | The field passes through tissue and induces small electric currents | Light is absorbed in the skin and the tissue just beneath it |
| Best-studied uses | Osteoarthritis pain, fractures that fail to heal | Joint and tendon pain, skin |
Does PEMF reach deeper into the body than red light?
They reach tissue in different ways: a magnetic field passes through the body and induces small electric currents, while light is absorbed as it travels through tissue. Depth describes how each works, not which one is better.
Bone research quotes fields of 0.1 to 20 gauss that produce 1 to 100 mV/cm in bone. Assiotis, 2012
Light works closer to the surface. In lab work on animal tissue, over 90% of near-infrared laser energy was absorbed within the first 10 mm, as our red light therapy for knee pain article sets out. Light trials therefore target joints near the skin, such as the knee and hand.
Inside cells, an enzyme called cytochrome c oxidase absorbs the light; see red light therapy for pain and inflammation.
Is PEMF or red light better for knee arthritis?
Both have placebo-controlled reviews that found less knee pain, and the reviews do not rank one above the other. NICE puts tailored exercise first for everyone with osteoarthritis NICE, 2022, so either therapy sits alongside that.
| Review | Therapy | What it pooled | Main finding |
|---|---|---|---|
| Cochrane, 2013 | PEMF vs placebo | 9 trials, 636 adults with osteoarthritis | 15.1 points more pain relief on a 0 to 100 scale; function not clearly different |
| Chen, 2019 | PEMF vs placebo | 8 trials, 421 people with knee arthritis | Physical function better; pain and stiffness not clearly different |
| Tong, 2022 | PEMF vs other conservative care | 11 trials, 614 people with knee or hand arthritis | Pain, stiffness and function improved |
| Stausholm, 2019 | Laser light vs placebo | 22 trials, 1,063 people with knee arthritis | Pain 14.2 mm lower overall, 18.7 mm lower at recommended doses |
| Elboim-Gabyzon, 2023 | PEMF vs laser light | 1 trial, 40 people | Pain and function improved in both groups |
The Cochrane authors concluded that PEMF "may provide moderate benefit for osteoarthritis sufferers in terms of pain relief". Cochrane, 2013 Other PEMF reviews found clearer gains in function than in pain. Chen, 2019 Vavken, 2009 Tong, 2022
For light, pain at recommended doses was 23.2 mm lower than placebo 2 to 12 weeks after treatment ended. Stausholm, 2019 No adverse events were reported. Most of these trials used clinic lasers; our knee pain article covers the smaller LED evidence.
What did the one trial comparing PEMF and laser on the knee find?
Pain and physical function improved in both groups after six sessions over three weeks. The trial was small, had no sham group and did not follow people after the three weeks.
The 40 people were aged 50 to 75 with moderate knee arthritis, and neither group did an exercise programme. Elboim-Gabyzon, 2023
- Laser group: 8 J/cm² at 5 points on the front of the joint line, 3 minutes per point, pulsed at 2 Hz.
- PEMF group: 30 Hz at 10 mT through two coils, 15 minutes per session.
- Both groups: twice a week for three weeks.
The PEMF group improved more on some measures, and no adverse events were reported in either group.
Does PEMF help broken bones heal, and is it the same as red light on a fracture?
PEMF bone stimulators are approved as a medical treatment for fractures that do not heal with standard care, and the orthopaedic team prescribes and supervises them. Red light on a fracture is a different thing: trials used minutes of light on bare skin after the cast came off, for comfort and movement.
A Cochrane review pooled 4 placebo-controlled trials in 125 adults whose long bone fractures had healed slowly or not at all. Cochrane, 2022 More fractures united with stimulation, but the difference was not statistically significant.
In one series of 44 people with slow-healing shin fractures, the stimulator was worn 3 hours a day for a median of 29.5 weeks, and 34 of the 44 fractures united. Assiotis, 2012 That series had no control group.
For fresh fractures, a 2024 review of 3 trials with 197 patients did not support PEMF for faster healing. Picelli, 2024
Our article on red light therapy for broken bones covers the light trials after the cast comes off. Never remove a cast, splint or brace yourself to use light.
Does PEMF or red light help pain after surgery?
PEMF devices are approved for post-operative pain and oedema in superficial soft tissue, and results in trials vary. Red light belongs to a later stage, once the wound has fully closed and the surgeon agrees.
NCCIH lists this post-operative use among PEMF's approved uses. NCCIH, 2023 In a sham-controlled trial of 118 adults after appendix surgery, a PEMF device worn over the dressing for 12 hours gave similar pain scores and painkiller use to sham. Perumal, 2022
For light, our guide to red light therapy after surgery explains the timing. Light never goes over stitches, staples or an open wound.
Can you use PEMF and red light therapy on the same day?
We found no published trial that tested PEMF and red light used together, so there is no evidence that combining them adds a benefit or that one should come first. They work by different means, so anyone using both should follow each device's own instructions and tell the clinician who suggested the PEMF device.
The nearest study tested a clinic device that combines light with a static magnet, which is not PEMF. Teixeira, 2022 In 72 adults with neck or shoulder pain, six sessions over three weeks lowered pain more than placebo up to 7 days after treatment. The design cannot show whether the light, the magnet or both made the difference.
In practice:
- One change at a time: keep your routine steady for a few weeks before adding the second therapy.
- Keep a log: note pain out of 10 and one task, such as stairs, each week.
- No set order: no tested order exists, so keep each session as its own manual describes.
Is a pulsed light panel or a magnetic bracelet a form of PEMF?
No to both. A panel's pulse setting switches light on and off, and static magnets have fields that "are always on and do not change", while PEMF uses electromagnets whose fields change in intensity.
Our panel can pulse its light from 1 to 10,000 Hz, which is flicker in the light itself. PEMF trials also give frequencies in Hz, but there the number describes how often the magnetic field changes. See pulsed vs continuous red light therapy.
Magnetic bracelets, insoles and mattress pads use static magnets. NCCIH states that research does "not conclusively support the use of static magnets for pain relief". NCCIH, 2023
How long are PEMF and red light sessions in the trials?
Light trials used minutes per session, usually over 3 to 6 weeks. PEMF trials ranged from 15 minutes twice a week for knee arthritis to 3 hours a day for months with bone stimulators.
| Study | Therapy and condition | Field or wavelength | Dose or time | Schedule |
|---|---|---|---|---|
| Stausholm, 2019 | Clinic laser, knee arthritis | 785 to 860 nm, or 904 nm | 4 to 8 J per spot, or 1 to 3 J at 904 nm | 8 to 12 sessions over 3 to 6 weeks |
| Elboim-Gabyzon, 2023 | Clinic laser, knee arthritis | Not stated, pulsed at 2 Hz | 8 J/cm², 15 minutes across 5 points | Twice a week, 3 weeks |
| Elboim-Gabyzon, 2023 | PEMF, knee arthritis | 30 Hz, 10 mT, two coils | 15 minutes | Twice a week, 3 weeks |
| NCCIH, 2023 | PEMF, chronic low back pain | Varied | Not given | 2 to 7 days a week, 1 to 12 weeks |
| Assiotis, 2012 | PEMF bone stimulator, slow-healing shin fracture | Worn over the limb | 3 hours a day | Daily, median 29.5 weeks |
NCCIH reports that in one osteoarthritis review, PEMF sessions shorter than 30 minutes worked better. NCCIH, 2023
Laser joules per spot do not convert into a panel session; what carries over is near-infrared light on bare skin, in regular sessions for several weeks.
Who should not use PEMF or red light therapy?
Cleveland Clinic advises against PEMF in pregnancy, with medicine given through a skin patch, and with epilepsy. Our precautions rule out red light during pregnancy, with epilepsy, with a thyroid condition or thyroid medication, with photosensitising medication, and over broken skin.
Cleveland Clinic also advises talking to a provider before buying a home PEMF device. Cleveland Clinic, 2026 NCCIH states that electromagnetic therapy appears "to be safe for treating pain", and that anyone pregnant or with a health condition should talk to a provider first.
Our panel manual adds these warnings:
- Pregnancy: not while pregnant, breastfeeding or preparing for pregnancy.
- Seizures: not with epilepsy or seizures triggered by light.
- Skin: not over open wounds, after sunburn on the body in the past 2 weeks, or with Fitzpatrick skin type V or VI.
- Sleep: not for use while asleep or by anyone unconscious.
- Eyes: goggles on and eyes closed with the panel, and never look into the light.
Our precautions rule out photosensitising medication, so anyone taking a medicine should check with their pharmacist or doctor before using the device.
How do you use a Kivorla panel or mask if you also use PEMF?
Use the panel for joints, the back and the neck, and the mask for facial skin; neither device has a PEMF function. Keep each device to its own instructions, and change one thing at a time so your log shows what helped.
The 9-Spectrum Red Light Therapy Panel carries 480, 590, 630, 660, 670, 810, 830, 850 and 1060 nm, each dimmable from 1 to 100%.
- Position: hang it on a door or wall with the included door hook, pulley, hanging cables and screws, or stand it upright on the floor.
- Distance: about 6 inches for a joint, about 12 inches for a broad area.
- Channels: favour 810, 830, 850 and 1060 nm for joints, on clean, bare skin.
- Timer: 10 to 15 minutes per area, set from 1 to 30 minutes with the controller. If no time is set, the panel stops at 5 minutes.
- Neck: side-on or from behind at neck level, goggles on, eyes closed, never aimed straight at the face.
- Rest: at least 6 hours between 30-minute sessions. The surface stays at or below 40°C.
The closest evidence comes from clinic lasers that gave small doses at a few points. To stay near that range, start with a shorter timer, turn the channels down from 100%, or step back from 6 to 12 inches, and do the same if your skin feels hot.
The 4-Wavelength Red Light Therapy Mask is a full-face silicone shell with an eye opening and no diodes over the eyes. Its red mode combines 665 nm red with 850 and 1064 nm near-infrared, and its blue mode combines 460 nm blue with 850 nm, set with the handheld controller or voice. The manual allows 10-minute sessions up to 4 times a week at high brightness, or 15-minute sessions up to 3 times a week, and never more than 20 minutes a day. The mask is for the face, not joints; the panel reaches the jaw and neck.
Read the precautions before ordering. Standard shipping to the US, UK and Germany is free. Under our return and refund policy, returns are accepted within 30 days: an unopened device with its seals intact is refunded less a 10% restocking fee, and an opened device that is undamaged and complete is refunded less 20%.
Kivorla devices are general wellness devices, not medical devices, and have no PEMF function. Ask your doctor or physiotherapist before adding any device to care for arthritis, a fracture or recovery from surgery.
