Red Light Therapy for Muscle Strain: Grades, Timing and Panel Set-Up
Red light therapy for muscle strain is best supported as a comfort add-on from about day 3, once first aid is done: in a review of trials in injured athletes, light lowered pain more than the comparison treatments. Further evidence comes from muscle damage trials, neck and back pain trials, and lab work. Below: strain grades, strain or soreness, timing, ice and heat, the hamstring, calf, back and neck, trial settings, warning signs and panel set-up.
Key takeaways
- A 2024 review of 6 randomised trials in 205 injured athletes found that light lowered pain compared with control treatments. Morgan, 2024
- In a placebo-controlled trial of 36 healthy men, laser light before hard thigh exercise kept a muscle damage marker at 436 IU/l at 48 hours, against 1,328 IU/l with placebo. Baroni, 2010
- The NHS advises protection, rest, ice, compression and elevation for the first 2 to 3 days after a strain, with no heat or massage. NHS
- A grade I strain usually heals in a few weeks, while a grade III strain is a complete tear that may need surgery. Cleveland Clinic
- Up to 10% of people with the symptoms of a calf pull have a blood clot in a deep vein, which needs a doctor first. Cleveland Clinic
Does red light therapy for muscle strain help?
The closest evidence says it can ease pain while a strain heals, used as an add-on to first aid and rehab. That support comes from trials in injured athletes, muscle damage studies in healthy adults and lab work on injured muscle.
The strongest human result is a 2024 review of 6 randomised trials in 205 athletes. Morgan, 2024 Light lowered pain more than placebo light, standard physiotherapy or a gel.
A university sports programme treated 395 injuries, strains included, with 830 nm LED light. Foley, 2016 In 65 athletes with complete records, return to play took 9.6 days on average, compared with an expected 19.23 days, not with a control group.
In rats with a freezing injury to a leg muscle, 780 nm light reduced inflammation on day 1 and increased new muscle fibres by day 7. Alves, 2014 These are lab results, not human strains. Our guide to red light therapy for injuries covers the wider research.
What do muscle strain grades I, II and III mean?
A strain is an overstretched or torn muscle, and the grade describes how much of it is torn. Grade I is minor, while grade III is torn all the way through.
Cleveland Clinic grades strains in three steps. Cleveland Clinic
| Grade | What is torn | Typical recovery | Where light fits |
|---|---|---|---|
| Grade I | Minor damage, no full tear | A few weeks | Add-on from about day 3, with gentle movement |
| Grade II | Some or most of the muscle; strength and range suffer | Several weeks to months | Add-on from about day 3, with the physio's programme |
| Grade III | Torn all the way through | May need surgery | The surgeon's plan first; light only once any wound has closed and the surgeon agrees |
Common sites are the hamstring, calf, groin, hip flexor, abdomen and back. A "pop" or a visible dent in the muscle points to a higher grade.
A grade 1 hamstring strain often improves in less than a week; grades 2 and 3 can take up to several months. Cleveland Clinic, hamstring
Scar tissue is "stiffer and more brittle than healthy muscle tissue", so a repeat tear comes more easily. Cleveland Clinic Strengthening is the main way to prevent the next strain.
Is it a muscle strain or normal soreness after training?
Normal training soreness starts 1 to 3 days after a workout and rarely lasts more than 5 days. A strain usually starts suddenly during activity, often with sharp pain, a pop, bruising or weakness.
Soreness lasting a week or more may point to a strain, as our article on red light therapy for muscle pain explains.
Training soreness has the most light research. A 2025 review of 14 studies found soreness lower than with placebo at 72 and 96 hours after exercise. Tsou, 2025 See what the research says about light and muscle recovery.
How soon can you use red light therapy for a pulled muscle?
From about day 3, once first aid is done and warning signs are ruled out. The first 2 to 3 days belong to protection, rest, ice, compression and elevation, with no heat, alcohol or massage.
The NHS advises ice "for up to 20 minutes every 2 to 3 hours" in that window. NHS Heat waits until swelling has gone down, at about 48 to 72 hours. Temple Health, 2025 A panel feels warm, so it follows the same timing.
The PEACE and LOVE framework says to protect the area for about 1 to 3 days, then load it early as symptoms allow. Dubois, 2019 Passive treatments do little early on, so light belongs beside the exercise plan, not in place of it.
| Phase | What the guidance says | Where light fits |
|---|---|---|
| Days 0 to 2 or 3 | Protection, rest, ice, compression, elevation | No light |
| Day 3 to week 2 | Gentle movement and early loading | 10 to 15 minutes per area, 3 to 5 times a week |
| Weeks 2 to 6 | Strength work; the NHS says avoid running for up to 8 weeks | Optional add-on; review after 2 to 3 weeks |
| Still painful at 4 to 6 weeks | See a physio or doctor | Follow their plan |
Mayo Clinic notes that "light movement is OK if it doesn't make symptoms worse". Mayo Clinic
Ice, heat or red light: which comes first for a strain?
Ice comes first, in the opening 2 to 3 days, and light comes later on its own occasion. Do not ice and light the same area back to back.
- Ice: 20 minutes out of every hour, several times a day, and "Do not apply ice directly to the skin". AAOS OrthoInfo
- Heat: after swelling has gone down, and not over an open wound or stitches.
- Light: from about day 3, once the skin has returned to its normal temperature and feeling after any ice. Warm is fine; if it feels hot, step back or stop.
Can red light help a pulled hamstring?
The closest evidence comes from trials of hard lengthening exercise on the thigh, the kind of load behind many hamstring strains. Light lowered muscle damage and strength loss in one trial and raised pain tolerance in another.
In the first, 36 healthy men had laser light or placebo, then did 75 maximal lengthening contractions of the front thigh muscles. Baroni, 2010
- Muscle damage marker: creatine kinase at 48 hours was 436 IU/l with light, against 1,328 IU/l with placebo.
- Strength: at 48 hours, 267 Nm with light, against 216 Nm with placebo.
The light came before exercise, in men without an injury.
In the second, 40 adults had 830 nm LED light or sham for 10 minutes on six sites over the front of the thigh. Chang, 2021 Pressure pain threshold was higher with light at 48, 72 and 96 hours, which the authors read as pain relief rather than faster repair.
For the hamstring itself, the physio's flexibility and strength work leads, and return to sport waits until they say it is safe. Cleveland Clinic, hamstring
Can red light help a calf strain, and how do you rule out an Achilles tear or a clot?
Light can sit alongside rehab for a calf strain once two look-alikes are ruled out: an Achilles tendon rupture and a blood clot. Both need medical help first.
A calf strain brings a snap or pop, sudden pain and swelling, and trouble standing on your toes, and takes several weeks to months to heal. Cleveland Clinic
- Blood clot: "up to 10% of people with symptoms of calf pulls have DVT". Cleveland Clinic One swollen, throbbing, red or darkened calf needs an urgent check.
- Achilles rupture: "a feeling of having been kicked in the calf", a pop, and trouble pushing off need help right away. Mayo Clinic
Once a simple strain is confirmed, treat one calf at a time at about 6 inches, the back of the calf toward the panel.
Does red light help a pulled back muscle or a strained neck?
The closest evidence comes from neck pain and low back pain trials, where clinic lasers lowered pain more than placebo. Most back strains improve within about two weeks, and neck strain often settles within a few days.
For the back, ice and rest for the first 24 to 48 hours give way to normal routines, because long bed rest delays recovery. Cleveland Clinic, back A 2015 review of 7 trials in 394 people with long-term low back pain found pain about 13.6 points lower on a 100-point scale with laser light than with placebo. Huang, 2015 Our article on red light therapy for back pain covers it in depth.
For the neck, a Lancet review of 16 trials in 820 people found pain 19.86 mm lower on a 100 mm scale with light than with placebo. Chow, 2009 In acute neck pain, people were 1.69 times as likely to report improvement with light.
- Lower back: panel centred at hip height, back to the panel, about 12 inches away.
- Neck: panel at neck level, side-on or from behind, about 6 inches from one side, never aimed at the face.
What settings did strain and muscle damage studies use?
Studies used red and near-infrared light from 660 to 950 nm on the skin over the muscle, in short courses. Most soreness trials used single sessions, and the athlete injury series used a few sessions per injury.
| Study | Who | Light | Course | Main result |
|---|---|---|---|---|
| Foley, 2016 (no control group) | 395 athlete injuries | 830 nm LED | Mean 4.3 sessions per injury | Return in 9.6 days, against an expected 19.23 |
| Chang, 2021 (sham) | 40 adults, sore thighs | 830 nm LED, 10 minutes | One session | Higher pressure pain threshold at 48 to 96 hours |
| Douris, 2006 (sham) | 27 adults, sore biceps | 660 and 880 nm LED | Daily for 5 days | Pain lower at 48 hours |
| Baroni, 2010 (placebo) | 36 healthy men | Laser | One session before exercise | Less muscle damage and strength loss at 48 hours |
Trial doses do not convert into panel minutes. What carries over is red with near-infrared, short sessions and a short course, and more light is not better, as our guide to how much red light therapy is too much explains.
Which pulled muscle symptoms need a doctor first?
See a doctor first if you heard or felt a pop with immediate weakness, cannot bear weight, or have severe pain or large bruising. Call 999 (or your local emergency number) if you heard a crack, the limb looks misshapen, or the skin is numb, blue, grey or cold.
- A pop and weakness: see a provider right away if you cannot move the muscle at all, or pain, bruising and swelling are severe. Cleveland Clinic
- Weight bearing: being unable to walk more than a few steps, worsening swelling or a very high temperature. NHS
- Back or neck: numbness down a limb, new weakness, or neck pain after a fall or crash.
- Broken skin: never use light over a graze, cut, stitches or any open wound.
Keep any brace or bandage your doctor or physio has asked you to wear.
How do you use a Kivorla panel on a muscle strain?
From about day 3, put the goggles on and place the strained area about 6 inches from the panel, or about 12 inches for a whole hamstring or the lower back. Run the red and near-infrared channels for 10 to 15 minutes per area, 3 to 5 times a week, alongside your rehab.
The 9-Spectrum Red Light Therapy Panel carries 480, 590, 630, 660, 670, 810, 830, 850 and 1060 nm, each dimmable from 1 to 100%. Its 660, 830 and 850 nm channels sit inside the range used in the muscle studies.
- Position: hang it on a door or wall with the included door hook, pulley, hanging cables and screws, or stand it upright on the floor.
- Bare skin: leggings, compression sleeves and bandages block most of the light.
- Channels: 630 to 850 nm on, 1060 nm optional, 480 and 590 nm off, continuous light.
- Timer: if no time is set, the panel stops at 5 minutes; the surface stays at or below 40°C.
- Spacing: at least 6 hours between 30-minute sessions; review after 2 to 3 weeks.
- Eyes: goggles on for every session, even for the legs, eyes closed, never looking into the panel. See our guide to red light therapy goggles.
- Log: pain out of 10 and what you can do, such as stairs or a slow jog.
The trials gave small doses to small areas. To stay near that range, set a shorter timer (from 1 minute), turn the channels down, or step back from 6 to 12 inches. Anyone with reduced feeling should start at 12 inches with short sessions.
Our precautions rule the panel out during pregnancy, breastfeeding or preparing for pregnancy; with epilepsy or seizures triggered by light; with a thyroid condition or thyroid medication; over broken skin; after body sunburn in the past 2 weeks; and for Fitzpatrick skin types V or VI. They also rule out photosensitising medication, so if you take any medicine, including ibuprofen or naproxen, check with your pharmacist or doctor first. The face mask is for the face, not muscles.
