Red Light Therapy for Shoulder Pain: Frozen Shoulder, Rotator Cuff and Setup
Red light therapy for shoulder pain has its best support as an add-on to shoulder exercises. In placebo-controlled trials on frozen shoulder, rotator cuff tendinitis and impingement, near-infrared laser light added to exercise lowered pain more than a sham device. Below: each shoulder problem, the trial doses, where to aim the light, warning signs, and how to set up a home panel at shoulder height.
Key takeaways
- In a frozen shoulder trial of 63 people, 810 nm laser plus exercise left pain 18.8 mm lower than placebo plus exercise at 4 weeks, on a 100 mm scale. Cochrane, 2014
- A 2026 meta-analysis of 10 trials on shoulder impingement found that light added to exercise lowered pain by 0.89 points on a 10-point scale. Photochem Photobiol, 2026
- After rotator cuff repair, people using an 808 nm light device at home for 3 months saw pain fall by 32 points on a 100-point scale, against 16 points with a sham device. Lasers Surg Med, 2023
- WALT dose guidance for the supraspinatus and infraspinatus tendons is 8 J in total over 2 to 3 points, with at least 4 J per point. WALT, 2010
- Mayo Clinic says frozen shoulder symptoms usually resolve within 1 to 3 years. Mayo Clinic
Does red light therapy help shoulder pain?
Research suggests it can ease shoulder pain, mostly when added to exercise. Pain is the most consistent gain, and several trials also found better movement and function.
Nearly every shoulder trial used a laser held over points on the skin, usually near-infrared. A 2016 Cochrane review of rotator cuff disease concluded that this light "may have short-term benefits over placebo in people with rotator cuff disease". Cochrane, 2016 Our guide to red light therapy for tendonitis gives the pooled figures from a 2015 meta-analysis of 17 shoulder trials. The table sorts the main evidence by shoulder problem.
| Shoulder problem | Main study | Light | Sessions | Result |
|---|---|---|---|---|
| Frozen shoulder | Placebo trial, 63 people, 2008 | 810 nm laser, with exercise | 12 over 8 weeks | 18.8 mm less pain at 4 weeks; better function at 4 months |
| Rotator cuff (supraspinatus) tendinitis | Double-blind trial, 24 people, 1995 | 820 nm laser | 9 | Less pain and weakness than a dummy laser |
| Impingement | Meta-analysis, 10 trials, 2026 | Mixed, with exercise | Varied | 0.89 points less pain out of 10; better range of motion |
| After rotator cuff surgery | Sham-controlled trial, 50 people, 2023 | 808 nm, self-applied at home | 15-minute sessions for 3 months | Faster pain relief at 3 and 6 months |
Does red light therapy help frozen shoulder?
Two small placebo-controlled trials suggest it can, the larger one with light added to exercise. A Cochrane review found that light plus exercise lowered pain in the first weeks and improved shoulder function for up to 4 months.
The review included 2 laser trials. Cochrane, 2014 In the larger, 63 people did 8 weeks of exercise with either 810 nm laser or a placebo laser.
- Pain at 4 weeks: 32.3 with laser against 51.2 with placebo on a 100 mm scale, a difference of 18.8 mm.
- Pain at 4 months: still 12.7 mm lower with laser.
- Function: 11.8 points better at 4 weeks and 13.8 points better at 4 months on SPADI, a shoulder pain and disability score.
- Night pain and pain on movement: both favoured the laser group.
That trial used 1.8 J on each of 8 painful points, 14.4 J per session, twice a week for 4 weeks and then once a week for 4 weeks. Photomed Laser Surg, 2008 In a second trial of 40 people, treatment over 6 days succeeded in 16 of 20 with laser and 2 of 20 with placebo. Neither trial reported side effects.
How long does frozen shoulder last, and where does light fit beside physio or a steroid injection?
Frozen shoulder usually runs its course over 1 to 3 years, so light is a way to ease the painful months, not a quick fix. Guidelines put pain relief, exercise and stretching first, with steroid injections for short-term relief, and light sits alongside that plan.
Mayo Clinic describes three stages: Mayo Clinic
- Freezing: about 2 to 9 months.
- Frozen: about 4 to 12 months.
- Thawing: about 5 to 24 months.
The NHS says "Frozen shoulder can take months or years to get better." NHS, frozen shoulder Its steps run from painkillers, to a steroid injection, to gentle exercises, and physiotherapy usually lasts at least 6 weeks.
The 2013 physiotherapy guideline gives its top grade to a steroid injection combined with stretching exercises, for better pain relief and function over 4 to 6 weeks. JOSPT guideline, 2013 An injection is a decision for your GP. Light is a home add-on to the exercise plan.
Does red light therapy help rotator cuff tendinitis and shoulder impingement?
At adequate doses, the trials point that way, mostly alongside exercise. The newest meta-analysis on impingement found less pain and better range of motion when light was added to exercise.
The 2026 review pooled 10 trials. Photochem Photobiol, 2026
- Pain: 0.89 points lower on a 10-point scale with light plus exercise.
- Range of motion: a mean difference of 12.24 in favour of light.
- Function scores: similar in both groups.
For the supraspinatus, the rotator cuff tendon on top of the shoulder, a double-blind trial of 24 people used 820 nm laser for 9 treatments. The laser group had less pain and less weakness than people given a dummy laser. Clin Rehabil, 1995
In a 2021 trial of 120 people with impingement, sessions ran 3 times a week for 8 weeks. Clin Rehabil, 2021 At 3 months, the median SPADI score had fallen from 60.8 to 2.3 with laser plus exercise, and from 61.5 to 14.2 with exercise alone.
Can red light therapy heal a torn rotator cuff, and can you use it after surgery?
Light has been studied for pain after a tear has been surgically repaired, not for closing a tear. In that setting, home sessions eased pain faster than a sham device.
The double-blind trial enrolled 50 people after keyhole rotator cuff repair. Lasers Surg Med, 2023 On top of standard care, they used a handheld 808 nm laser device at home in 15-minute sessions for 3 months, at 16.5 J/cm².
- Pain at 3 months: fell by 32 points on a 100-point scale with light, against 16 with sham.
- Pain at 6 months: fell by 41 points against 23.
- Meaningful improvement at 3 months: 76% with light, 48% with sham.
- Strength and movement scores: similar in both groups, so the gain was faster pain relief.
After surgery, use light only with your surgeon's or physiotherapist's agreement, and never over an unhealed incision.
A new tear needs a doctor first. The NHS advises an urgent GP appointment or NHS 111 for sudden or very bad pain, pain after a fall or injury, an arm you cannot move, a shoulder that has changed shape or is badly swollen, lasting numbness, or feeling feverish. NHS, shoulder pain See a GP if the pain is getting worse, or is no better after 2 weeks.
Should you use red light before or after shoulder exercises?
Either side of your exercises fits the research, as long as the exercises stay. Nearly all positive shoulder trials added light to an exercise programme rather than replacing it.
In the 2021 impingement trial, the laser plus exercise group did best on function. Clin Rehabil, 2021 Some impingement trials with strong exercise programmes found light added little, so keep the exercises as the base.
As our article on light and muscle recovery notes, there is no settled answer on timing, so a session on the same day as your rehab, before or after it, fits the research. The NHS suggests shoulder exercises for 6 to 8 weeks.
Red or near-infrared: which light reaches the shoulder?
Near-infrared is the band to lead with for the shoulder. The positive shoulder trials covered here used 808 to 820 nm, and the rotator cuff sits deeper than the elbow tendons.
Because the shoulder structures are deep, WALT sets higher doses here than for the elbow: WALT, 2010
- Supraspinatus and infraspinatus: 2 to 3 points, 8 J in total, at least 4 J per point.
- Shoulder joint: 2 to 4 points, 8 J in total, at least 4 J per point.
- Schedule: daily for 2 weeks or every other day for 3 to 4 weeks.
Our comparison of 660 nm and 850 nm light explains the difference in depth.
Where should you point the light on a sore shoulder?
Start with the front and side of the shoulder, then turn to light the top and back of the shoulder blade. Together these cover the areas the trials treated.
The frozen shoulder trial treated each person's 8 most painful points. A 2015 study of 50 frozen shoulders in older adults listed its target areas: J Pain Res, 2015
- Top: the space under the bony tip, where the supraspinatus runs.
- Front: the front of the joint capsule and where the biceps tendon attaches.
- Underneath and back: the lower pouch towards the armpit, and the back of the capsule.
The WALT rows add the infraspinatus, over the back of the shoulder blade. Use bare skin.
What doses and schedules did the shoulder trials use?
Laser on points, usually 2 or 3 times a week for up to 8 weeks, alongside exercise or usual care. Doses were a few joules per point, or 16.5 to 30 J/cm² where reported that way.
| Study | Wavelength | Dose | Time | Sessions per week | Weeks |
|---|---|---|---|---|---|
| Frozen shoulder, 2008 | 810 nm | 1.8 J per point, 14.4 J per session | 30 seconds per point | 2, then 1 | 8 |
| Supraspinatus, 1995 | 820 nm | 30 J/cm² | Not stated | 9 sessions in total | Not stated |
| Impingement, 2021 | Not in abstract | Not in abstract | Not stated | 3 | 8 |
| After surgery, 2023 | 808 nm | 16.5 J/cm² | 15 minutes | Not stated, at home | 12 |
| WALT guidance | 780 to 860 nm | 8 J, at least 4 J per point | Varies | Daily, or every other day | 2 to 4 |
These were lasers held over small points, and laser joules per point do not convert directly into the joules per square centimetre a broad panel delivers. The pattern to borrow is the rhythm: regular sessions each week for several weeks, with the exercises kept up.
Can you use red light for frozen shoulder if you have diabetes or a thyroid condition?
With diabetes, check with your GP or diabetes team first, since diabetes can slow frozen shoulder recovery. With a thyroid condition or thyroid medication, do not use Kivorla devices at all.
The 2013 guideline reports that diabetes raises the risk of frozen shoulder about 5 to 6 times, and that people with diabetes may recover more slowly. JOSPT guideline, 2013 In a small trial of 40 people with type 2 diabetes and frozen shoulder, laser 3 times a week for 8 weeks, with exercise, improved all four shoulder movements measured. Asian J Pharm Clin Res, 2014
Mayo Clinic lists both overactive and underactive thyroid among frozen shoulder risk factors, and the thyroid sits close to the light field of a shoulder session. Our precautions also rule out use during pregnancy, with epilepsy, with photosensitising medication, or over broken skin, and the panel is not for Fitzpatrick skin types V or VI. No side effects were reported in the Cochrane frozen shoulder or rotator cuff laser trials. Skin should feel warm, never hot; stop if it becomes irritated.
How do you set up a Kivorla panel at shoulder height?
Hang the panel on a door or wall so its centre lines up with the sore shoulder, then sit side-on with near-infrared leading. Use the dimmer, timer and distance to keep the session close to the studied range.
The 9-Spectrum Red Light Therapy Panel carries 480, 590, 630, 660, 670, 810, 830, 850 and 1060 nm, each dimmable from 1 to 100%. Its 810, 830 and 850 nm channels sit close to the 808 to 820 nm used in the shoulder trials. At 6 inches it puts out at least 1,567 W/m², with near-infrared at least 1,200 W/m².
- Height: hang it on a door with the door hook, or on a wall with the screws, and use the hanging cables and pulley to bring its centre level with your shoulder. Check the hook and pulley are secure first, as our guide to setting up a panel at home explains.
- Position: sit side-on to the panel, then turn to light the back of the shoulder blade.
- Channels: lead with 810, 830 and 850 nm, and turn the others down.
- Eyes: wear the goggles and keep your eyes closed.
Our general routine for a joint is 10 to 15 minutes per area at 6 inches, three to five times a week, judged at three to four weeks. The shoulder trials used modest doses on small points, so to stay near the studied range, bring the dose down:
- Dimmer channels: turn near-infrared and red down from 100%.
- Shorter timer: it starts at 1 minute, and the panel stops at 5 minutes if no time is set.
- More distance: step back to 12 inches, our distance for a broad area such as the shoulder and upper back.
Keep the same settings for 3 to 4 weeks, with at least 6 hours between 30-minute sessions. Rate your pain from 1 to 10 before, after and the next morning, and note how high you can lift the arm. Goggles, a controller, a door hook, a pulley, hanging cables and screws come in the box. The 4-Wavelength Red Light Therapy Mask covers the face only.
Standard shipping to the US, UK and Germany is free. Read the precautions before ordering.
